List of Special Education Forms

1. The form CAN BE FILLED OUT ONLINE. However, please note that you cannot submit this form electronically. You will need a printer to print the form. IMPORTANT be sure to: SIGN and DATE your form.

2. You are accountable for all Education allowance and related care funding. Members are accountable and responsible for making schooling, education & family reunion travel applicable payment on behalf of the student. Notwithstanding, Outside Canada Support units have been authorized to make the payment of admissible education expenses directly to an educational institution on behalf of a member or group of members for entitled education benefits as outlined in the Post Ceiling and FSD 34. The member is accountable and obliged to inform Outside Canada Support unit in writing, immediately on receipt of a bill from a school and/or when a child terminates schooling during the academic year for which an education allowance has been paid. Member is to seek in writing, when applicable, any funding which may be refunded by the school under such circumstances must be made payable to the Department of National Defence. In the event that the school, in error, makes a refund directly to the member, this must immediately be refunded to the Department of National Defence. Members are not to take the “Tuition Refund Insurance” as this item would be a “non-admissible” education expense. All original receipts are to be forwarded to your Outside Canada Support Unit.

3. Outside Canada because of school location, unavailability of professional services, language of host country and school population may not provide special education services as available within provincial special education policies. To assist, when a special education requirement is identified while the student is enrolled outside Canada, the parent may refer to:

FORM 11 SPECIAL EDUCATION REQUIREMENTS

PARAMETERS Submission to include:
  1. Recommendations by appropriate school authority AND appropriate professional personnel.
  2. Source and qualifications of remedial assistance person and estimated number of hours/cost per hour required for remedial assistance must be provided.
  3. Information on whether funding for health care, if applicable, has been expended.
  4. It is the responsibility of the member to provide documentation in support of the special education allowance, such as, but not limited to, an assessment and recommendation of appropriate professionals prior to submission to the appropriate Foreign Service interdepartmental coordinating committee.
INTENT When proper education screening has been completed prior to posting outside Canada and special education needs are identified and remedial resources are available at the Post to provide a member with resources to meet provincial standards.

FORM 30 EDUCATION ALLOWANCE - SPECIAL AFNORTH/SHAPE SCHOOL REQUIREMENTS

PARAMETERS When AFNORTH or SHAPE Canadian School authorities have requested special service not provided by the school, such as:
  • Psycho- Educational Report as completed by Clinical Psychologist , Brussels
  • Follow up professional services not provided by the school and recommended Clinical Psychologist
  • Speech Therapy
  • Other
INTENT When proper education screening has been completed prior to posting outside Canada and special education needs are identified and remedial resources are available at the DND Dependants Schools Overseas to provide a member with resources to meet provincial standards.
